What the inspector saw: Chicken, beans, shredded beef, salsa and enchilada sauce stored at unsafe temperatures; cockroach found inside a cooking pot stored with clean dishes; hand-washing sink lacked soap, paper towels and hot water; kitchen equipment was not sanitized.
Follow up: Passed a second re-inspection June 13.
