• The restaurant received a “fail” after a May 29 inspection when the inspector noted eight critical violations, including an active roach infestation with insects inside dry ingredient containers, the ice machine and other areas of the kitchen.

• What the inspector saw:

• Eight critical violations on its June 8 follow-up, when many of the violations from the previous inspection hadn’t been corrected. There were still some roaches, but the inspector noted in her report that it was understood that it could take more than 10 days to minimize their presence.

• Passed June 19.
