Kentucky-based Kindred Healthcare owns eight facilities in Arizona. Three are in Tucson, including the Villa Campana Health Care Center, which the state ordered to pay a $10,000 fine this summer.
One of its other local facilities - Desert Life Rehabilitation and Care Center, 1919 West Medical St. - also was fined by the state this summer. Desert Life was ordered to pay $4,500 for failing to provide supervision to a resident who sustained second- and third-degree burns while smoking, state officials said.
Desert Life failed to identify and correct "deficient practice for residents who smoke," the state report says. The state also faulted Desert Life for failing to apply interventions with two residents with known cognitive impairments, and for failing to ensure that interventions were in place to prevent further risk of harm to 12 residents.
Officials with Desert Life say they filed a plan of correction with the state and stressed that they are now in full compliance with all state and U.S. regulations.
