The Star will publish articles over the next several months to shine a light on medical errors, with a focus on hospitals, nursing homes and assisted living facilities.
By the end of the series we will publish toolkits for consumers to use, whether you are signing paperwork for a loved one to enter an assisted living facility, find yourself in an emergency room, or a family member is hospitalized.
And we would like patients, doctors, nurses and other readers to give us guidance by filling out one of two questionnaires. All responses are confidential unless you say otherwise.
Information from the online-only questionnaires will be submitted to and stored with ProPublica, an independent nonprofit newsroom based in New York that produces investigative journalism, including an ongoing project on patient harm.
